I can't look at a map of Ukraine without feeling slightly melancholy. I sure do miss it there, I also wish I could see Ukraine in the spring. I hear it is very beautiful, though we thought the snowy scenes were breathtaking.I think one of the most confusing aspects to adopting is the huge piles of paperwork you need before, during and after the adoption. Remember when you had a baby and all you did was fill out that birth certificate in the hospital?
Well, we knew we wanted Kara to have a Arizona State Birth Certificate of Foreign Birth, and we had help learning how, it is not easy to find, and not many folks seem to know about it. I thought I would share the links in our blog for other families, that way they don't need to look for it like we did. :o)
For Arizona families, this is what we did: (copied and pasted)
What Do You Need to Register a Foreign Born Birth Certificate?
If the person was adopted in the foreign country by both parents, the following documents are required to register a birth certificate.
IR-3 Stamped Passports
A Foreign-Born Birth Certificate will be created for a child adopted from a foreign county by an Arizona resident if the following documents are provided:
1. A copy of the passport page that shows the IR-3 stamp.
2. The Adoption Decree
Note: Evidentiary documents submitted in a language other than English must have an English translation of the evidentiary document prepared by a certified translator and have an official authentication stamp. Additional legal documents such as the birth certificate, abandonment certificate may be requested if sufficient information is not included in the adoption decree.
3. Fee ($10.00 includes one certified copy of registered birth certificate, additional copies are $10.00 each)
4. The foreign-born worksheet
5. A completed and signed foreign-born birth application using the adoption information.
6. A copy of the adoptive parent’s valid government-issued photo identification with a signature.
